For an adult age 18+ in the United States preparing the next step for their own ordinary U.S. passport. Identify which situation applies before choosing a report or application.
Identify the situation
State’s loss-reporting instructions distinguish:
- Valid physical document lost or stolen: report promptly. Reporting invalidates it even if later found; it does not replace it.
- Expired document or exposed number only: do not report this as valid physical-document loss.
- Passport never received by mail: follow the separate status/nonreceipt instructions. Missing citizenship originals are another mailing.
Privately establish product, validity, previous report and actual receipt. Unknown facts remain unresolved; publish no passport identifiers or scans.
Separate the next steps
The page offers online reporting through its official Form Filler, mailed signed DS-64 with both sides of photo ID copied, or reporting during an in-person application. Online cancellation says within one business day; other paragraphs say “immediately.” Infer no instant or individual guarantee.
A report confirmation does not establish a replacement application, appointment or new passport.
For replacement, State’s adult application instructions require the in-person route. Its form section says to include the recent passport’s loss circumstances in DS-11, plus a police-report copy if filed; insufficient details can pause processing for DS-64. Check the complete current packet and travel-timing route separately. Ordinary acceptance facilities and urgent agency appointments differ; availability and issuance remain unconfirmed. This guide excludes abroad, minors, disasters and another person’s passport.
